Easier access to Family Health Center’s mental health and addiction services

You no longer have to wait for your first behavioral health evaluation appointment. By visiting our Access Center, you can walk in and take the first step towards your mental health needs.

We provide timely screening, assessment and connection to the appropriate level of care based on each person’s needs.

The Access Center is available to individuals seeking help with:

  • Mental health concerns
  • Substance use concerns
  • Emotional or behavioral difficulties
  • Family or relationship concerns
  • Changes in functioning at home, school, work or in the community
  • Urgent behavioral health needs
  • Crisis concerns

Frequently Asked Questions – Clients

Do I need a referral?

No. Individuals may contact us directly. Referrals may also come from family members, physicians, hospitals, schools, community agencies, law enforcement or other providers.

What happens when I contact Open Access?

Our access team will:

  • Listen to your concerns.
  • Gather information about your current needs.
  • Complete appropriate screening and risk assessments.
  • Identify immediate clinical or safety needs.
  • Determine the appropriate level of care.
  • Connect the individual with the appropriate service or provider.
  • Provide referrals or resources when services outside our organization are needed.

What information should I bring?

If available, bring:

  • Photo identification
  • Insurance information
  • Medication list
  • Information about current providers
  • Relevant medical or behavioral health information
  • Guardianship or legal documentation, when applicable

What if I do not have insurance?

You should still contact us. Staff can explain available options, eligibility requirements, financial assistance and other resources. The Family Health Center does not refuse service to anyone based on their ability to pay.

What if I don't know what service I need?

That's okay! You don't need to determine the service yourself. Our trained staff will help identify the most appropriate service based on your needs.

Can children and adolescents use the Access Center?

Yes. Parents, guardians, caregivers, schools or other authorized referral sources may assist with accessing services for children and adolescents.

Can someone refer a family member or another person?

Yes. A referral source can contact us to discuss making a referral. Privacy and confidentiality requirements may limit what information we can provide back to the referral source without appropriate authorization. Please keep in mind that all Access services are voluntary, and the individual must agree to receive services.

What If I'm having a crisis?

Tell the access staff immediately if you are experiencing a safety or behavioral health crisis. Staff will assess the situation and help connect you with the appropriate crisis response.

If there is an immediate danger to you or someone else, call 911 or go to the nearest emergency department.

Will I receive services the same day?

Our Access Center model is designed to provide timely access to care and in almost all cases, you will receive services the same day. Depending on your needs, same-day services may include screening, assessment, crisis intervention, safety planning, brief intervention, referral or connection to ongoing services.

What happens after I am seen at the Access Center?

You may be connected to:

  • Therapy services
  • Medication services
  • Case management
  • Care coordination
  • Peer support
  • Substance use services
  • Crisis services
  • Community resources
  • Or other appropriate levels of care depending on your needs

Is my Information confidential?

Yes. Your behavioral health information is protected by federal and state confidentiality requirements. Information is shared only when permitted by you or required by law and applicable authorization requirements.

Can my school, doctor, probation officer or another agency receive information about my treatment?

Information generally cannot be shared simply because someone requests it. When appropriate, a valid authorization or other legal basis may be required before information can be disclosed, subject to applicable laws and confidentiality requirements.

What if the services I need are not provided here?

Our staff will help connect you with an appropriate provider or community resource whenever possible. The Access Center is intended to help people get connected to the right care - even if it is not a service we provide.

How do I make a referral?

Provide available information regarding the individual's current needs, symptoms, safety concerns, medications, substance use, recent hospitalizations, current providers and other relevant information.

What if I think the person is in crisis?

Clearly communicate the concern. Do not submit an urgent or emergency concern as a routine referral. Notify the access team of known safety risks or significant changes in functioning by calling us directly and speaking to a member of the team.
